Class 1 – Who we are and what we do

Practice details, information about our Practice team and our opening times are all published on our website. This information can be accessed via the About Us page. 

Class 2 – What we spend and how we spend it

The Practice receives money from NHS Wales according to its contract for national General Medical Services in exchange for services provided for patients.

There may be circumstances where material cannot be released because it is confidential or commercial information or the appropriate officer designated for these purposes, under the Act, has taken the view that it may be prejudicial to the conduct of the Practice’s affairs.

If this is the case, we will respond to your FOI request with a formal letter acknowledging the reasons why we are unable to give you this information.

Class 3 – What our priorities are and how we are doing

Working closely with our colleagues in the Arfon Cluster to develop and improve services for patients in the local area.

Working towards achieving required standards such as QUAIF Assurance Framework (QUAIF), Access Standards, Medicines Management, Patient Safety, IG Toolkit and Clinical Governance Practice Self-Assessment Tool ( CGPSAT)

Working with our neighbouring practices and the Health Board to continuously improve the services that are offered within Llanberis Surgery including facilities improvements and ongoing maintenance of the building. 

Class 4 – How we make decisions

The partners will meet periodically with the Practice Manager to review practice plans and policies.

Class 5 – Our policies and procedures

Our policies and procedures are published on our website. These can be accessed via the Practice Policies page.

Class 6 – Lists and Registers

We hold patient information in our clinical system called EMIS which is held on secure servers managed by the NHS. Within EMIS we operate registers which are used to monitor our provision of care, record clinical data and manage recalls.
